A. Couples living together for five (5) years or more without the benefit of marriage
1. Couples should be present for the signature of Joint Affidavit for Article 34;

1. Applicants for marriage license below 25 years shall bring with them their parents for signature of consent and/or advice;
2. Birth/Baptismal Certificate
3. Residence Certificate of applicants
C. Unregistered Marriages
1. Three (3) pcs. xerox copy of Marraige Contract
2. Residence Certificate of applicant couple (new)
3. Name and Date of Birth of Children
Requirements & Procedures on requesting for Death Certificate

1. For current death - secure Death Certificate from the hospital (if died in the hospital) and the City Health Office (if not in the hospital)
2. For Late Registration of Death: Secure Certification of Death from Barangay Captain
3. Secure Certification of no record of death from LCR
4. Proceed to the City Health Office for accomplishment of Death Certificate
5. Back to the LCR for registration
